There’s a lot of important information reported to the IRS on your 1095-B Forms regarding the offers of health care coverage you made throughout the year. Included in the information you’re required to provide is the
origin of the policy, which is entered on Line 8 of Form 1095-B.
When you come to Line 8 of Form 1095-B, you’ll enter a letter (A-F) to indicate where the health insurance policy you offered your recipients originated:
- A. Small Business Health Options Program (SHOP)
- B. Employer-sponsored coverage
- C. Government-sponsored program
- D. Individual market insurance
- E. Multiemployer plan
- F. Other designated minimum essential coverage (MEC)
